Wednesday – 8:30am
to Noon
Monday & Friday – 8:30am to 5:30pm

WHAT
TO BRING WITH YOU
- When
visiting the Health Center for the first time, you must bring proper
identification (i.e. Social Security card, valid driver’s license,
birth certificate or another form of official identification).
If you have private insurance, Medicaid or Medicare, you must bring your
card.
- If
uninsured, you have to bring a pay stub, an unemployment check stub,
a recent tax return or a letter from your employer to prove your
family income.
- You
must also bring all current medications with you.
- Children
needing immunizations must have their current record of immunizations
received.

TO SCHEDULE AN APPOINTMENT
To
schedule a medical appointment, please call 405.632-6688.

schedule a dental appointment, please call 405.632.6688 ext. 267.
OCHS does accept same-day appointments.
OCHS
only accept patients on a walk-in basis between the
hours of 8:30AM-11AM and 1PM-3PM (except on Wednesdays).
Your wait time may be longer due to scheduled appointments
being seen first. It is OCHS policy that new
patients to the health center are unable to be seen
on a walk-in basis. A new patient appointment
must be scheduled first, then after the initial visit,
the patient can be seen on a walk in basis.
For
those living in rural southwestern Oklahoma, you can contact
our Fort Cobb Health Clinic or our Tipton Health Clinic to make
your appointments.
